In 2015, the Quality Institute created the Conversation of Your Life (COYL) program, which provides mayors and community partners with tools to develop a community conversation around end-of-life care planning in a culturally sensitive and accessible way. Now there are 11 counties across the state that convene county task forces including: Bergen, Burlington, Camden, Gloucester, Hudson, Hunterdon, Mercer, Middlesex, Monmouth, Somerset, Union.
